So it looks like the most important consideration is who we are getting a transplant from or who we are giving it to and what are the karmic consequences of our choices.
Everyting in this world belongs to Krishna. So you cannot donate something thats not yours.
On the practical side, donate it. It belongs to Krishna, Just as you have used Krishna's property,
so also others may enjoy it, so they too can see the image of the Infinite.

First thing is that we are not this body. We are spirit soul trapped in this body and when this body becomes old or useless we change our bodies. Krishna is the properiter of everything so if we donate eyes to someone it is not against any scriptures instructions. In ancient times also people gave up their body organs. For example a sage called Dadhichi gave up his body and out of his bones "Vajra-the weapon of Indra" was made to kill the demons. Also everyone is Krishna's children so if we do something for other people then that is Krishna's service though indirect service.
The only thing is that we will have to bear its Karmic reactions whatever good or bad.
This is answer according to my understanding.Please correct me if i m wrong.

When one is a devotee, and you are constantly chanting the Holy Names, should we pay any attention to karma like this?
Isnt chanting the Holy Name once, even with offenses more than sufficient to achieve liberation?

Yes you are right about karmic reaction.
How ever, if you use what ever you are donating in the service of the Lord then you are not bound by karma.
Perhaps you can make the recipient look at Krishna's image every one hour(or what ever frequency),
and only under such condition they can receive the eyes...
Under such circumstance, you are using your eyes only to serve the Lord... in making someone a devotee.
