What is the 'far many things' that the person has to do? Work, take care of family and social responsibility? If the body is only not there, what work will the person do? Jaan hai to jahan hai - we dont even need to delve into spirituality to get the answer.
We have to take care of the body, because only the human body is the instrument to understand the purpose of life and to attempt to go back home, back to godhead.

I think it depends on the illness and the costs for treating the illness. I recently discovered that I do not have breast cancer. While waiting for those test results, I was looking up the costs for treatment. I do not have "good insurance" and I would have had to pay more money for surgery than I have in the bank. And for what? I see people paying lots of money for cancer surgeries and other cancer treatments and then the cancer comes back anyway. I do not want to be homeless. But some people have become homeless because of huge medical bills that completely wipe them out.
Hare Krishna Prabhuji Dandvat!!!!! first of all kindly expand the meaning of your not caring properly. If your mean is that you are not taking your food on time or your feel guilty while wearing the nice clothes or if you will go for morning walk and keep yourself very dashing and will wear the branded outfits then please do not feel guilty. This body is given by Lord Krishna to us so we need to respect it. Since we are all in karma bhumi so we need to fulfill all our karma while keeping the Lord Krishna in centre. Suppose if we are going to office so we will have to be ready as per the dress code given by the company but at the same time we need to always remember Lord Krishna in our heart and need to thanks Lord Krishna. Lord Krishna devotee should be very proactive while doing their daily karma and at the same time they need to fix them in their Bhakti Sadhna. So just keep yourself very active so that you can set the example for non-devotees that one can be very active in their daily duties while fix in the devotional service of Lord Krishna. It depends on the consciousness of the person. Are you neglecting your body and not taking care of it because you are so absorbed in Krsna that you dont even remember whether you ate or bathed or not? If that is the case, then it is not a sin.
Any other matter - pls examine why you are not taking care of the body.
Pls remember that only in the human form of life, we have the discriminating power to understand that this world if full of miseries and temporary and that the real purpose of life is to go back home, back to godhead. How can you concentrate on your sadhana if you are in a sick body which needs constant care and attention? Would it not be a good idea to take basic care of your body such that it doesnt come in between your sadhana, without being paranoid and cooking up diseases in your mind.
