PAMHO.
Offering to Gaur Nitai can be done in the same plate. There is no necessity for two separate plates.
Whatever standards of worship you set in your home, you have to follow.
Tulsi will be at the feet of Gauranga only.
I had occassion to be in a group which was asking HG Jananivas Prabhuji about deity worship rules when they had got Deities in their house and I am writing what I heard him say:
In a temple, the standard of worship is followed because the temple is the house of God, everything is as per His timings. However, in a household, God adjusts according to the timings and standards of whatever house He is in. But once a standard has been set, it is ideal to follow that standard. Therefore, it is advisable for the standard to be barest minimum in households. Therefore, if you want to do abhishek everyday, you can do so. Else, it is advisable that every ekadasi you do the abhishek. You have to see that your deities are looking fresh and are glowing. Accordingly, you can set the standard. Regarding bhoga also, the group had deities in different floors of their house and different deities. They wanted to know if they had to do offerings in each floor separately and plate for each deity separately. Prabhuji said not necessary, do as per convenience.
He mentioned that it is mentioned in the scriptures that women should not touch the deities in their contamination period. However, if a woman has taken a vow to take care of her deities all her life, that vow is bigger than the scriptures. Generally, in India, joint families are the norm and there is more than one lady in the house. Therefore, if one cannot do, the others do. But in nuclear families, where there is no alternative, and there is noone to take care of the deities those days, and it is completely unavoidable, then it may be permitted. Still it depends on the level of consciousness of the mataji. If she is too much in the bodily conception, then she should not touch. But if she is of the consciousness that she is not the body, she is the soul and that she is serving the deities who are God Themselves, then she can touch provided she maintains high level of cleanliness and bathes regularly.

i use one plate for both of Them.
If garlands are not fresh (if they are made of real flowers) then You have to change. Otherwise it is not comfortable to use not fresh flowers, isn't it?!
Usually i try to offer Tulasi Manjaris and leaves to Their feet. And put some of them in Their cup of water.
