    Death is the changing of the body by the soul. In the Bhagawad Gita (2.13) , it is mentioned

    dehi nosmin yatha dehe, kaumaram yauvanam jara,

    tatha dehantir praptir, dhirastatr na muhyati.

    The way a soul travels through boy hood, youth and old age, that same way, the soul travels from one body to another, a dhira is not bewildered by such changing of body.

    Rituals done after death of a relative are as prescribed by the vedas, therefore they have scriptural backing and are authentic. They are designed to help the soul's onward journey after leaving the present body.

    The journey of the soul after death is defined by the karma of the soul - the destination has again been given in the Bhagawad Gita - Anta kale ca mam eva, smaran muktva kalevaram and the next shloka - whatever one desires at the time of death, that yoni one attains in the next birth. Therefore, the soul leaves one body when the next body is decided. That next body is decided based on karma of this birth. To remember God at the end of our lives, we have practise remembering Him all our lives, because we dont know whihc moment will be the last moment.
