if pure devotee of lord knows that he isnt this body, how its possible that he suffers from body ?
pure devotee always takes shelter and is fully depend on lotus feet of lord,
so how is possible that lord dont protect his pure devotee as he protect arjuna on the battlefield ?
is it ever happened in history of universe that pure devotee suffers from illness ?

smṛta-mātrārti-nāśanaḥ
Word for word:
kāśyasya — of Kāśya; kāśiḥ — Kāśi; tat-putraḥ — his son; rāṣṭraḥ — Rāṣṭra; dīrghatamaḥ-pitā — he became the father of Dīrghatama; dhanvantariḥ — Dhanvantari; dīrghatamasaḥ — from Dīrghatama; āyuḥ-veda-pravartakaḥ— the inaugurator of medical science, Āyur Veda; yajña-bhuk — the enjoyer of the results of sacrifice; vāsudeva-aṁśaḥ — incarnation of Lord Vāsudeva; smṛta-mātra — if he is remembered; ārti-nāśanaḥ — it immediately vanquishes all kinds of disease.
Translation:
The son of Kāśya was Kāśi, and his son was Rāṣṭra, the father of Dīrghatama. Dīrghatama had a son named Dhanvantari, who was the inaugurator of the medical science and an incarnation of Lord Vāsudeva, the enjoyer of the results of sacrifices. One who remembers Dhanvantari can be released from all disease.

Why would the pure devotee of the Lord, doesn't wish to leave the material body?
In fact, he would be least bothered about the material existence and would wish to leave the current physical form as early as possible to reach the Supreme Lord's abode.
And coming to illness, yes, it is possible and it is due to the karma of a person and the devotee is ready to suffer and doesn't wish for any recovery since he always knows that he is not this material body, but the soul.

Pure devotee of the lord is in the body which is subject to the 4 defects - birth, death, old age and disease. How is it that the pure devotee will be exempt from disease alone, but not from the other 3?
Like they say - pain is inevitable, suffering is optional. Pure devotee also has disease, only he is not affected by it like a normal person. He is very much interested in praying to Krsna and doing loving devotional service that he disregards the bodily pain and suffering and continues regardless. Example in modern times in front of our eyes is HH Jayapataka Swami Maharaj. He is having so many bodily sufferings, but he continues to preach, do his Saligram seva, take care of devotees. He completely disregards his pain and continues with his devotional service. That does not mean that he will not go to doctor or take medicines to cure himself - he does all of that, but his devotion is primary.
