Wear only the small one then. When we wear tulsi beads, it means that yama dutas will not come to take us at the end of our life, Vishnu dutas will come. Since we dont know which is our last moment, once we wear the beads, we always wear them. If the bigger beads are bothering you at night, you may consider wearing the smaller beads at all times.
I read in, I believe the Bhakti Yoga Handbook, that Srila Prabhupada said that one should wear Tulasi Neckbeads always and never remove them. Even of one is naked, sleeping, in the bathroom or even passing stool. They should be worn at all times.
