So to apply Tilak to 12 places and pronounce the Holy Names helps us to remember Krishna even while taking care of this body. And reminds us that this body is a Temple of Lord Krishna and should be used for His pleasure.
In Bhakti Rasaamrita Sindhu it says that to mark the Holy Names on the body is one type of devotional service so it is one of them.
And also this body is a tiny copy of a real Virat Rupa of the Lord. So in those 12 places reside those 12 incarnations of Lord Vishnu. So we apply Tilak there.
To apply Tilak and to keep cleanliness really helps us to be in Krishna Consciousness - in proper consciousness. It makes us strong - which means helps us to control our lower desires. So it is very important to follow Vaishnava etiquette.
