Medicines in Ekadasi

Yes, anything that is medically necessary can be taken even on Ekadasi.
This is because the higher principle than to observe Ekadasi 100% strictly is to do whatever it takes to keep body, mind, and everything else fit for service to Krishna.

The scriptural context is explained by Rupa Goswami as follows:

anasaktasya vishayan
yatharham upayunjatah
nirbandhah krishna-sambandhe
yuktam vairagyam ucyate

prapancikataya buddhya
hari-sambandhi-vastunah
mumukshubhih parityago
vairagyam phalgu kathyate

"When one is not attached to anything, but at the same time accepts everything in relation to Krishna, one is rightly situated above possessiveness. On the other hand, one who rejects everything without knowledge of its relationship to Krishna is not as complete in his renunciation." Bhakti Rasamrita Sindhu 2.255-256

In the purport to BG 6.10, Srila Prabhupada writes:

"A Krishna conscious person well knows that everything belongs to Krishna, and thus he is always free from feelings of personal possession. As such, he has no hankering for anything on his own personal account. He knows how to accept things in favor of Krishna consciousness and how to reject things unfavorable to Krishna consciousness."

Body belongs to Krishna, therefore, if we hanker to maintain 100% strict Ekadasi, but if that means we cannot serve Krishna as intensely or as long as possible, then we are responsible for not using Krishna's property in the service of Krishna properly. In other words, one would be more attached to observing Ekadasi for personal gain than to serving Krishna.
